Agreements

I, {Dog Owners Name}, assume full responsibility and liability for any injuries, damages, or incidents caused by {Dogs Name} to any person, property, or other animals while on the premises of State Street Brats.

I agree to keep {Dogs Name} on a leash or in a secured carrier at all times and under my control while on the patio.

I will ensure {Dogs Name} remains well-behaved and does not disturb other patrons or their pets.

I understand that no dogs are allowed on the patio after 9 PM.

I acknowledge that dogs are not permitted on tables, chairs, or benches, per Wisconsin Food Code 6-501-115, and I will avoid having {Dogs Name} on my lap at a table.

I certify that {Dogs Name} is/are up-to-date on all vaccinations and is in good health and I agree to provide proof of vaccination upon request by State Street Brats staff.

I will clean up after {Dogs Name} and dispose of waste properly.

I release State Street Brats, its owners, employees, and affiliates from any liability, claims, or expenses arising from my {Dogs Name}'s presence on the patio.

I agree to indemnify and hold harmless State Street Brats for any claims, damages, or expenses, including legal fees, resulting from any actions of {Dogs Name}.

I understand that State Street Brats reserves the right to ask me and {Dogs Name} to leave for any reason, including but not limited to noisy, aggressive behavior, or lack of control over {Dogs Name}. This decision is at the sole discretion of the restaurant.

I understand that I am responsible for {Dogs Name}'s actions and behavior at all times while on the premises of State Street Brats.
